In December 2025, the previous Health Secretary Wes Streeting ordered a Statutory Public Inquiry into TEWV NHS Foundation Trust. This follows multiple failures in patient care that tragically resulted in preventable deaths and failures to provide appropriate treatment.
The inquiry aims to review past failings in relation to both inpatient and outpatient mental health care, failures in structures, reporting of problems and accountability and how meaningful change can be implemented to ensure the public have faith in the provision of mental health services within the region.

Current Status of the Public Inquiry
Latest Update:
- 1st June 2026: the Secretary to the Inquiry was appointed
- 18th June 2026: a meeting of the families affected and their legal representatives was held with the Secretary
Despite assurances from Wes Streeting that this inquiry would be progressed with urgency, the process has since stalled, leaving those affected to face ongoing uncertainty while new allegations of avoidable harm within the Trust continue to emerge.
At present, those affected are still awaiting the appointment of an inquiry chair, despite assurances that this appointment would be made by the end of February. Whilst there have been expressions of interest from Judges to become the Chair, there is still no formal roadmap in place.
